This website is operated by Revivo Neurology Treatment Centre ("Revivo", "we", "us"), 525 Markham Road, Unit 4, Scarborough, Ontario M1H 3H7. We handle personal information in line with Canada's Personal Information Protection and Electronic Documents Act (PIPEDA) and, where personal health information is involved, Ontario's Personal Health Information Protection Act (PHIPA).

Please don't include detailed medical records in a web form; we'll gather what we need during your consultation.
Like most websites, our hosting provider may automatically log basic technical information such as IP address, browser type and pages visited, for security and to keep the site running properly. Embedded videos are loaded from YouTube's privacy-enhanced mode only when you choose to play them, and the map on our contact pages is provided by Google.
How we use your information
