The focus of most post-stroke treatment programmes is to enable the patient to reassume his/her independent life and to recover normal body function and strength to the greatest possible extent.
This depends in part on how much and which part of the brain tissue has been affected by the stroke. However, in many cases doctors recommend a rigorous rehabilitation programme that may require the specialist attention of neurologists, dieticians, psychiatrists, speech, occupational, and recreationaltherapists, nurses and other rehabilitation specialists.
Stroke rehabilitation programmes are usually as intensive and demanding as individual patient circumstances permit, particularly during the first six months when the greatest amount of recovery generally takes place. Rehabilitation focuses on improving the stroke patient’s ability to perform functional tasks and on strengthening muscular movements in weakened joints and other parts of the body.
One way to achieve this is to “force” the patient to use their weakened limb by constraining the unaffected limb for up to 90% of the time. This means that the stroke victim has no choice but to use the affected limb to complete desired tasks – which can be both psychologically and physically challenging in the beginning but which with persistence and repetition may get gradually easier the more the task is performed.
Elongation of affected muscles is also an intrinsic part of stroke rehabilitation therapy. Repeated stretching and rotational movements of stroke-affected muscles can help to increase their recovery and prevent muscle contraction and ‘wasting’ from occurring.
Persistent and repetitive physical therapy and encouragement are key if the stroke patient is to achieve permanent lasting improvement in their affected motor skills.
Once the immediate crisis is over, the sooner physical treatment is started after a stroke occurs and the more intensive and continuous it is, the greater the chances for recovery. It is often considered that the windowof opportunity for optimal recovery is during the first six months following a stroke, but in fact every person’s stroke recovery is different and some people continue to further recover their motor skills in terms of improving walking, running, writing and talking for many months after this.
